We are pleased to announce the offering of a comprehensive medical and vision
benefit package through Independence Blue Cross/Keystone Health Plan East
for all full time employees working 30+ hours a week

There are three medical plans you can choose from and the chart on the following page
illustrates the three options you have along with the monthly cost to participate in
the plan. These costs already factor in the 25% contribution the agency
is paying out towards the total cost. Monthly rates are based on age so please refer to the
following pages to determine your monthly contribution per plan.

If you are interested in participating in a plan, you will need to complete an application
and submit it to Human Resources by the provided deadline.

If you are not interested in participating in a plan, you will need to complete a waiver
form and submit it to Human Resources by the provided deadline. Please note if you are
not taking the plan it is very important to check the refusal box - other reasons & explain
why. Acceptable responses are if you have State/Medicaid coverage, coverage through a
a spouse or another job.
We will need a completed application or waiver form from all full time employees so
you cooperation in submitting these forms is greatly appreciated.
