PA Waiver Services Factsheet

Support Services Waiver -- or simply waiver -- is a shortened term for the Medicaid Home and Community Based Waiver Program. This program provides funding for supports and services to help you to live in your home and community. Waivers offer an array of services and benefits such as choice of qualified providers, due process and health and safety assurances.

The name waiver comes from the fact that the federal government "waives" Medical Assistance/Medicaid rules for institutional care in order for Pennsylvania to use the same funds to provide supports and services for people closer to home in their own communities.

 

. The newest Living Independence for the Elderly ProgramIn Pennsylvania, the Department of Human Services administers 10 Medical Assistance/Medicaid waivers and thewaiver, addresses the needs of adults living with autism.

 

PA Consolidated Waiver (0147.R05.00)

Provides education support, home and community hab (unlicensed), homemaker/chore, licensed day hab, prevocational, residential hab, respite, supported employment-job finding/support, supports coordination, nursing, therapy services, supports broker services, assistive technology, behavioral support, companion, home accessibility adaptations, specialized supplies, transitional work, transportation, vehicle accessibility adaptations for individuals w/IID ages 3-no max age

 

PA Person/Family Directed Support (0354.R03.00)

Provides education support services, home and community hab (unlicensed), homemaker/chore, licensed day hab, prevocational, respite, supported employment-job finding/support, supports coordination, nursing services, therapy services, supports broker, assistive technology, behavioral support, companion, home accessibility adaptations, specialized supplies, transitional work, transportation, vehicle accessibility adaptations for persons w/IID ages 3-no max

 

PA Attendant Care (0277.R04.00)

Provides personal assistance, service coordination, community transition, participant-directed community supports, participant-directed goods and services, PERS for individuals w/PD ages 18-59

 

PA HCBW for Individuals Aged 60 & Over (0279.R04.00)

Provides adult daily living, personal assistance, respite, service coordination, home health services, accessibility adaptations/equipment/technology/medical supplies, assistive technology, community transition, home adaptations, home delivered meals, non-medical transportation, participant-directed community supports, participant-directed goods and services, PERS, specialized medical equipment and supplies, teleCare, therapeutic and counseling for aged individuals ages 65 - no max age and individuals w/PD ages 60-64

 

PA AIDS (0192.R04.00)

Provides home health services, specialized medical equipment and supplies, nutritional consultation, personal assistance services to individuals w/HIV/AIDS ages 21 - no max age

 

PA COMMCARE (0386.R03.00)

Provides personal assistance, prevocational, respite, service coordination, structured day habilitation services, supported employment, home health, adult daily living, assistive technology, community integration, community transition, non-medical transportation, PERS, residential hab, specialized medical equipment and supplies, therapeutic and counseling, vehicle mods for individuals w/brain injury ages 21 - no max age

 

PA Independence (0319.R04.00)

Provides adult daily living, personal assistance services, respite, service coordination, supported employment, home health, assistive technology, community integration, community transition services, home adaptations, non-medical transportation, PERS, specialized medical equipment and supplies, therapeutic and counseling, vehicle mods for physically disabled individuals ages 18 – 59

 

PA Medicaid Waiver for Infants, Toddlers and Families (0324.R03.00)

Provides habilitation to IID/DD children ages 0-2

 

PA Adult Autism (0593.R02.00)

Provides day habilitation, residential habilitation, respite, supported employment, supports coordination, therapies, assistive technology, career planning, community transition services, family support, home modifications, nutritional consultation, specialized skill development, temporary supplemental services, transitional work services, vehicle modifications for individuals w/autism ages 21 - no max age

 

PA OBRA (0235.R04.00)

Provides adult daily living, education services, personal assistance, prevocational, residential hab, respite, service coordination, structured day hab, supported employment, home health, FMS, accessibility adaptations/equipment/technology and medical supplies, assisted technology, community integration, community transition, home adaptations, non-medical transportation, PERS, specialized medical equipment and supplies, therapeutic and counseling, vehicle mods for individuals w/DD ages 18-59
